Nestlé UK has been fined £180,000 over the death of a worker in its Halifax chocolate plant. The fatal accident occurred in December 2008, when a worker switched on a depalletizer that had been switched off by another worker so he could remove a blockage.

An investigation by the UK’s Health and Safety Executive found that a safety key device used to stop the machine and ensure that it could not be started while maintenance was in progress was available, but Nestlé had failed to instruct workers as to its purpose and use.
